• Is not a medical device
• Is not an emergency response service
• Does not replace doctors, hospitals, or emergency services

3. User Accounts & Eligibility

• You must be 18 years or older, or
• A lawful caregiver managing health information for another person
• You are responsible for all activity under your account
• You agree to provide accurate and current information

4. User-Provided Health Information

All health information on Eliva:

• Is entered or approved by users or their caregivers
• Is not verified by Eliva
• May be incomplete or outdated

Users are responsible for:

• Accuracy of information
• Keeping records updated
• Reviewing shared data before use

5. Emergency Medical Card Disclaimer

The Emergency Medical Card:

• Displays user-provided information
• Is intended only as a reference
• Must not be relied upon as the sole source of medical information
